Description
第一形態において、本発明は、患者の身体内の特定された治療部位において解剖学的な中空管を熱切除するのに適合する装置であって、
末梢先端部を含む末梢端と、基部端と、少なくとも細長い本体長さの部分に沿って伸張し、予め配置されたガイドワイヤーの上に装置をスライド可能に搭載するために形成された中央内腔と、を有する細長い本体を含み、
末梢先端部は、中空管の腔内閉鎖を起こすある温度に中空管の壁を加熱することが出来る少なくとも一つの加熱モジュールを含み、少なくとも一つの加熱モジュールは細長い本体の末梢先端に配置される第一電極と、第一電極の基部方向の位置に配置される第二電極と、を含み、第一電極及び第二電極は、7mm以上15mm以下の間隔で離間されている二極性無線周波数(RF)電極装置を含み、
末梢先端部はさらに、中空管の壁に接触して及び/または貫通するように細長い本体から外方向に展開されることが出来る少なくとも一つの延長可能な要素を含む装置を提供する。
以下、本発明を理解するために参考になる発明を第二形態〜第四形態に示す。
末梢端に隣接してかつすぐ近位の基部の位置に配置された第一RF電極を含む第一末梢先端部を含む末梢端と、基部端とを有するガイドワイヤーと、
第二RF電極を有する第二末梢先端部を含む末梢端と、基部端と、少なくとも細長い本体の長さの部分に沿って伸張し、ガイドワイヤー上に細長い本体をスライド可能に配置するために形成された中央内腔と、を含む細長い本体と、を含み、
RFエネルギーの適用において、第一RF電極及び第二RF電極は中空管の腔内閉鎖を起こす温度へ中空管の壁を加熱させるために協同することが出来るように、使用時にガイドワイヤーと、細長い本体と、は並置される装置を提供する。
末梢先端部を含む末梢端と、基部端と、少なくとも細長い本体長さの部分に沿って伸張する中央内腔と、を有する細長い本体を含み、
末梢先端部は、遠隔に配置された電極と協同し、中空管の腔内閉鎖を起こすある温度に中空管の壁を加熱することが出来る単極性RF電極を有し、
RF電極は、その長さが2mm〜20mmの間である装置を提供する。
(a)患者の身体内の予め決められた部位から離れた部位において、末梢先端を有し、末梢先端を予め決められた部位の付近内の位置に実質上向けているガイドワイヤーを血管内へ導入する工程と、
(b)そこに配置された少なくとも一つの加熱モジュールを有する末梢先端領域を含むカテーテルをスライド可能な搭載によってガイドワイヤー上に導入する工程と、
(c)ガイドワイヤーに沿ってカテーテルを追尾することによってカテーテルの末梢先端領域を患者の身体内の予め決められた部位へ向ける工程と、
(d)血管の腔内閉鎖を起こす点へ組織が加熱されるように加熱モジュールを経て血管の壁にエネルギーを加える工程と、
(e)(d)工程内のエネルギーの適用を監視する工程と、
(d)腔内閉鎖が完了した時、エネルギーの適用を終了する工程と、
(e)閉鎖された血管からカテーテルとガイドワイヤーを回収する工程と、
を含む血管によって供給される組織内の障害の部位内あるいは組織内の障害の部位に近接する患者の身体内の予め決められた部位における血管の腔内閉鎖の方法を提供する。
他に述べることなしに、ここで使用される単語は同業者によって理解されるのと同様の意味を有する。全ての引用文献はここではそっくりそのまま参考文献に加入される。
装置は、発電機にアダプターケーブルを経て接続され、最小動力ワット量は、1−40Wの間のワットを用いてウシ肝臓組織内に多数のカテーテルを適用することにより、決定された。これは5ワットに決定された。
電極距離の変動:図20(a)は、ウシ肝臓組織内のカテーテル先端の二極性構造の2つの変体によって得られた凝固組織のパターンを示す。RFエネルギー5ワットにおいて、末梢電極と基部電極の間の距離が10mmの所で、分離した異なった切除集中点が得られたことを見ることが出来る。(5ワットで)電極間距離が7mmの所で、切除集中点は、単一の細長い切除領域を与えるように集まる。単極性構造の結果が、遠隔の接地パッドに補足された電極に搭載されたカテーテルと共に図20(b)に示される。広範な切除領域が矢印Gによって示される。
Claims (12)
- 患者の身体内の特定された治療部位において解剖学的な中空管を熱切除するのに適合する装置であって、
末梢先端部を含む末梢端と、基部端と、少なくとも細長い本体長さの部分に沿って伸張し、予め配置されたガイドワイヤーの上に装置をスライド可能に搭載するために形成された中央内腔と、を有する細長い本体を含み、
前記末梢先端部は、前記中空管の腔内閉鎖を起こすある温度に該中空管の壁を加熱することが出来る少なくとも一つの加熱モジュールを含み、該少なくとも一つの加熱モジュールは該細長い本体の該末梢先端に配置される第一電極と、該第一電極の基部方向の位置に配置される第二電極と、を含み、前記第一電極及び前記第二電極は、7mm以上15mm以下の間隔で離間されている二極性無線周波数(RF)電極装置を含み、

- 前記延長可能な要素は、ワイヤー、アーム、パネル及び針からなる群から選ばれる請求項1〜3のいずれかに記載の装置。
- 前記二極性無線周波数(RF)電極装置は更に、前記中空管の壁に接触および/または壁を貫通するように前記細長い本体から外方向に延長される少なくとも一つの前記延長可能な要素を含む請求項1〜4のいずれかに記載の装置。
- 前記末梢先端部は、前記少なくとも一つの延長可能な要素をそれによって押さえる引き込み可能な外部さやによって覆われることが出来、前記装置が正確に患者の身体内の特定された部位に配置されると、前記少なくとも一つの延長可能な要素を外部に向かって延長させるように、前記外部さやが基部に向かって引っ込められることが出来る請求項5に記載の装置。
- 前記延長可能な要素は、予め圧力を加えられた電気伝導性材料からなり、そのため押さえられていない状態で、該延長可能な要素は、該装置の該細長い本体の縦軸から外に向かって延長する請求項5または6に記載の装置。
- 前記延長可能な要素は、金、プラチナ、銀、金属合金、ニチノールのような形状記憶合金、ステンレス綱、及びチタニウムのうちの一つから選ばれる材料からなる請求項5〜7のいずれかに記載の装置。
- 前記延長可能な要素は、前記中空管の該壁に貫通するように該細長い本体の縦軸から外に向かって伸びることが出来る少なくとも一つのアームからなり、前記少なくとも一つのアームは、その変形温度が腔内閉鎖がおこる温度あるいはその付近の温度であるように形成される形状記憶合金からなり、該変形温度に達すると、該少なくとも一つのアームの配列は、該細長い本体の縦軸から外側へ伸びるものから、該細長い本体の縦軸に実質上平行であるものへと変化する請求項8に記載の装置。
- 前記末梢先端部の基部の位置において該細長い本体上に配置され、該中空管の一時的な閉塞を起こすように該細長い本体から外に向かって拡張されることが出来る拡張可能な閉塞構造物をさらに含み、前記拡張可能な閉塞構造物は、嚢内への、液体あるいは気体の膨張流体の導入によって膨張させることが出来る嚢を含む請求項1〜9のいずれかに記載の装置。
- 前記解剖学的な中空管は血管である請求項1〜10のいずれかに記載の装置。
- 前記熱切除は、前記解剖学的な中空管の閉塞を起こす請求項1〜11のいずれかに記載の装置。
