Add dep, delete unused files, update import paths

This change:

• adds dep dependency management
• updates import paths to use the gradienthealth fork
• deletes unused files
• adds verbosity to dicomutil
• sets up our tavis ci badge

and generally takes stock of the repository and cleans minor things up.

For now I've kept us setup on travis for this, but it's also possible to set up jenkins for public repos which we can do if we want faster builds and consistent build infrastructure in the future.
