  1. run shell-format on explicit file save 

    added configuration `shellformat.runOnSave` to toggle the possibility
to automatically run the command `shell.format.shfmt` when explicitly
saving the file.

added package-lock.json, as per recommendation[0]:
"This file is inteded to be committed into source repositories"
