fix: image insertion bugs

Fixes:

• when inserting image using the image file picker, the image-preview cursor was being reset
• in the same flow, the image was being added to frame that was currently in the center of viewport at time of file picker open. This is incorrect, and it should add the to frame on pointerdown only.
