This field is not part of the openapi-spec.
The code has been entirely copied from the edited file!
This effectively reverts 11306f9 As of Digitaler-Impfnachweis/certification-apis#63 only one vaccination may be added per certificate.
This will only be displayed, if there is no calendar input for dates.
The form will be extracted to a separate file.
Code for getting the form has been copied from the edited file.
