Implement on window title change hook + README revisions

• Implement window title chnage hook (window signal name-changed)
• Format as MD and minor revisions

Only issue is the name-changed signal is often (though not always) emitted twice. Could you see if you can reproduce on your system?

Tested on:

Debian GNU/Linux trixie/sid

with:

libgtk-3-0t64:amd64          3.24.41-4
libwnck-3-0:amd64     43.0-3+b1
libglib2.0-0t64:amd64             2.78.4-7
liblua5.4-0:amd64   5.4.6-3+b1 (though the double firing is not at the Lua level)
