Improve Markdown output

Many small fixes that Improves the layout of the generated Markdown documents.

• Removes the extra empty row on all tables
• Fixed some spacing issues in headers
• Always generate a new line at end of file
• Fixed badges to display a space instead of '+' if multiple words
• Added alt text for image badges
• Allowed the format annotation to be displayed
• Fixed the ToC generation when multiple files are converted at once (e64b49f)

Also fixed a spelling issue for Pattern Properties

Generation of examples have been put into a separate commit for ease of reviewing.
