New Data Model and CSS Fixes. #32

This PR introduces the following major changes:

Modify data model in firebase to store chapters as nested objects instead of sub collections.
Closes Server Render components on the last chapter page. #31
Closes Add publish date to individual chapters for sitemap. #30
Closes Introduce Header Image on chapter pages. #29
Code Cleanup and maintenance.
