This PR updates the derived concepts to work for MIMIC-IV v2.0. It also fixes some incompatibilities in trying to run the
convert_bigquery_to_postgres.shscript on Mac OS X as it has BSD sed and bash 3.x by default (as opposed to GNU sed and bash 4.x+ on Ubuntu).Almost all concepts are materially unchanged, but file changes exist, as the schema prefix has been changed from
mimic_tomimiciv_. The only real change is fromethnicitytoracein the icustay_detail table.
