Added R data wrangling file for the tidying of the Part B .xlsx file, as well as a data dictionary for the clean file (found on data.world)

Adding in an R file used to tidy the .xlsx Medicare Part B spending file, and which was also used to upload the tidied data to data.world. Also adding in data dictionary for the Part B spending data
