Description
この種のエンドプロスシーシス(endoprosthesis)は、通常、気球によって伸張できる単一な金属網状物で形成されている。或いは、自動伸張ができるように形成されたものである。管腔の中を通って拡張処理を行った後、エンドプロスシーシス(endoprosthesis)は、探針を使うことによって管腔内に挿入され、そして伸張され又は取り外される。
しかし、上記のような従来のエンドプロスシーシス(endoprosthesis)は、完全に満足のものとは言えない。即ち、筋肉組織が網状物の空隙に入り込み、外傷を起こさせるだけではなくて、血流における乱流をも起こさせている。
更に、従来のエンドプロスシーシス(endoprosthesis)は、動脈瘤の治療に使用できず、そして液密性ではないために、一般に、脈管などの管状器官の二つの健康段節を連結するときに適用できない。
本発明は、上記の欠陥を解消できる伸張性エンドプロスシーシス(endoprosthesis)を提供することにある。この目的を達成するために、本願発明は、下記の特徴を具備している。
即ち、上記網状物は、展延性及び生体適合性を有するプラスチック又はエラストマーのフィルムに覆われ、該フィルムが実質的にエンドプロスシーシス(endoprosthesis)の全体にわたって、網状物の空隙を充填すると共に、網状物の表面全体をカバーしていることを特徴としている。
本発明によれば、上記フィルムが、ポリウレタン等のポリマー、若しくは天然ゴム又は合成ゴムで形成されている。
エンドプロスシーシス(endoprosthesis)は、自動伸張型のものであって、伸張状態で端部が張り出すように形成されている。
上記網状物は、ステンレススチール又は、予め放射線不透過性になっている。テトラフルオロエチレン等の比較的剛性のあるプラスチックで形成されている。
本発明におけるもう一つの目的は、上述したような自動伸張可能なエンドプロスシーシス(endoprosthesis)を装着するための用具を提供することにある。
この用具は、下記の部材を備えている。
即ち、収縮状態のエンドプロスシーシス(endoprosthesis)を収容するチューリップ状部材を末端部に備えたガイドチューブと、
上記チューリップ状部材を長手方向に開放する手段と、
を備えている。
一つの実施態様によれば、上記手段は、上記チューリップ状部材を幾つかの花びら状部材にカットするワイヤを含み、当該ワイヤは、操作用ハンドルに接続されている。
他の実施態様によれば、
上記手段は、チューリップ状部材における長手方向の開口部を有し、チューリップ状部材の各側縁部は一連のガセットを備え、両隣接側縁のガセットは互いに係合されていて、これらを通過して操作用ハンドルに接続されているコードによって保持されている。
以下、図面を参照しながら本発明の実施例を説明する。
図1は、収縮状態にある本発明のエンドプロスシーシス(endoprosthesis)を概略的に示すものである。
図2は、伸張状態にある上記エンドプロスシーシス(endoprosthesis)を概略的に示すものである。

図1及び図2に示したエンドプロスシーシス(endoprosthesis)1は、フィルム3に覆われた管状の網状物2によって形成されている。
網状物2は、生体適合性のステンレススチール製である。網状物2は、管状物を軸方向に覆うようにしてヤーンを織り又は編むことによって作られ、或いは他の適切な方法で作ることができる。網状物2は、可塑的に変形でき、その軸と平行に延びた菱形を形成する空隙がある図1に示した小径の第1安定形状、及び円周方向に延びた菱形を形状する空隙がある図2に示した大径の第2安定形状をとる。
網状物2は、その空隙を塞ぐ展延性及び液密性の材料でできたフィルムに完全に埋められている。この材料の展延性は、網状物の空隙の変形に拘らず、収縮状態から伸張状態に至る網状物2の変形にフィルム3が破断や離脱なく追従できるために十分である。好適な材料としては、天然ゴム,合成ゴム等のような生体適合性のあるエラストマーや、ポリウレタンのような生体適合性のあるポリマーが掲げられる。
フィルム3による網状物2のコーティングは、金属材を脱脂してプライマー接着剤で処理した後、複合押出し又は浸漬法によって行われる。
伸張状態(図2を参照)で、液密性の管状セグメントは、管腔の中を通っての拡張処理を行った後、エンドプロスシーシス(endoprosthesis)又は「伸展器」として、使われることが可能である。筋肉組織や血液が、円滑なエラストマー表面又はポリマー表面と接触しているので、エンドプロスシーシス(endoprosthesis)は、筋肉組織に外傷を起こさせることがなく、血流の中の乱流も起こさせない。
漏洩性がないことから、このエンドプロスシーシス(endoprosthesis)は、エンドプロスシーシス(endoprosthesis)を動脈瘤につなぐようにし、各端部を、動脈瘤と隣接している健康動脈環節の内壁に放射状に当てることにより、動脈瘤の管腔内治療に使用できる。
図3乃至図7に示した他の実施例においては、エンドプロスシーシス(endoprosthesis)1Aの網状物2は、自動伸張可能で、バネ性を有するステンレススチールを使って従来の方法で作られたものである。
装着に際し、エンドプロスシーシス(endoprosthesis)1Aは、半径方向の圧縮により図1に示した非安定な形状になり、図3乃至図5に示した用具5のチューリップ状部材4に挿入される。
チューリップ状部材4の末端部は、開放状態にあり、互いに120度の角度を形成する三つの切欠き6を有する。その基端部は、ガイドチューブ8が延びる内側肩部7を形成する。各切欠き6の平面において、チューブ8の肉厚部を通って形成された通路9は、肩部7近傍の一端及びチューブ8の基端部の他端で、放射状に配列されたオリフィス10,11を経て外方に現れている。
また、図示のように、チューブ8の肉厚部を通って流体注入用の長手通路12及び13を設けることが可能である。通路12及び13は、当該チューブの基端部から始まり、肩部7付近にあるチューブ8の内部空間に現れる。
前述した各平面それぞれにおいて、可撓性ワイヤ14は、切欠き6を通過する。該ワイヤ14の内側ストランド15は、チューブ状部材4の内部に沿って延び、肩部7に設けられているオリフィス16を経て、オリフィス10を通過し、通路9に沿って延び、オリフィス11を経て、操作ハンドル17(図3)につながっている。ワイヤ14の外側ストランド18は、チューリップ状部材の外壁に沿って延び、ストランド15と同じ通路9,11を経て、ハンドル17につながっている。従って、このハンドル17は6本のワイヤとつながっていて、3本の内側ストランド15はエンドプロスシーシス(endoprosthesis)1Aの伸張傾向によって、チューリップ状部材の内壁に対し押しつけられたフラットな状態にある。
エンドプロスシーシス(endoprosthesis)を使用する前、まず、管腔の中を通っての拡張処理を行い、用具5を糸で一つのガイド部材につなぎ、次いで皮膚を通過して挿入し、希望する部位に応じて管腔内へ導入する。
その後、施術者は、ハンドル17を引く。これによって3本のストランド15が引っ張られ、各内側ストランド15が立体を作り出すそれぞれの母線に沿ってチューリップ状部材をカットする。チューリップ状部材は、エンドプロスシーシス(endoprosthesis)から徐々に離れる。図6に示すように、エンドプロスシーシス(endoprosthesis)は、自動的に伸張してくる。チューリップ状部材が完全に開いたとき、チューブ8を引くことによって当該用具が取り外される。
伸張状態(図7)で判るように、エンドプロスシーシス(endoprosthesis)の両端部側は、自動的に外方へ張り出した状態から見られ、それによって二つの有利な効果が奏される:その一は、エンドプロスシーシス(endoprosthesis)及び動脈からの漏洩がないことを確保できる;その二は、網状物2を形成するヤーンの端部19は、フィルム3を超えて若干延び、エンドプロスシーシス(endoprosthesis)を動脈に固定する多数の固着点を形成する。このようにして、エンドプロスシーシス(endoprosthesis)が、位置的に安定化する。
網状物2を形成するために他の材料を使用しても良い。例えば、自己伸張可能なエンドプロスシーシス(endoprosthesis)を製造するため、予め放射線不透過処理したバネ特性を持ち比較的剛性のあるポリテトラフルオロエチレン(PTFE)等のポリマーのヤーンが使用される。
用具5の他の例を、概略的に図8に示す。この用具5は、チューリップ状部材を長手方向に開放する手段において、前述した用具と相違する。
チューリップ状部材は、その高さ全体にわたって、所定の線に沿って長手方向に切り開かれている。スロットの各側縁は、一連の突出している円筒状ガセット20を有している。チューリップ状部材が閉じられた円筒状態にあり、自己伸張可能なエンドプロスシーシス(endoprosthesis)1Aを収縮状態に保持しているとき、隣接する両側のガセット20が互いに係合し、全てのガセットを通り基端部で操作ハンドル17に接続されているコード21によって全体が保持される。
エンドプロスシーシス(endoprosthesis)は、ハンドル17を引くことにより簡単に開放される。
Claims (8)
- 自動伸張可能なエンドプロスシーシス(endoprosthesis)を装着するために用いられる用具であって、
末端部を有するガイドチューブと、
自動伸張可能なエンドプロスシーシス(endoprosthesis)を収容可能にし、前記ガイドチューブの末端部に設けられ、チューリップ形状を有する円筒状収容部と、
自動伸張可能なエンドプロスシーシス(endoprosthesis)に影響することなく前記収納部を長手方向に開ける収納部開放手段と、
を備えたことを特徴とする用具。 - 請求項1記載の用具であって、前記収納部開放手段は、操作用ハンドルと、前記収納部を複数の部分にカットするために操作用ハンドルに接続されているワイヤとを有することを特徴とする用具。
- 請求項1記載の用具であって、前記収納部は、収納部内の側縁を形成する長手方向の開口部を有し、更に各側縁に沿って配置された一連のガセットを有し、各側縁に沿っている一連のガセットは、互いに接近して装着されており、
前記収納部開放手段は、操作用ハンドルとコードとを有し、このコードは、各側縁を互いに接近させて保持するように各側縁に沿っている一連のガセットを通過しており、前記コードは作動用ハンドルに接続されていることを特徴とする用具。 - 請求項2または3記載の用具であって、前記収納部は、チューリップ状の収納部分を有することを特徴とする用具。
- 請求項4記載の用具であって、前記ガイドチューブは、流体注入用の少なくとも一つの長手方向の溝部を有することを特徴とする用具。
- 自動伸張可能なエンドプロスシーシス(endoprosthesis)を装着する用具であって、
収縮状態のエンドプロスシーシス(endoprosthesis)(1A)を収容するチューリップ状部材(4)が末端部に設けられたガイドチューブ(8)と、
上記チューリップ状部材を長手方向に開放する手段(14,17;20,21,17)と、
を備えている用具。 - 前記手段(14,17)は、操作ハンドル(17)に接続され、チューリップ状部材(4)を複数の花びら状部材にカットする複数のワイヤを備えていることを特徴とする請求項6記載の用具。
- 前記手段(20,21,17)は、個々の縁部に一連のガセット(20)があるチューリップ状部材の長手方向開口部をもち、二つの側縁にあるガセット(20)が互いに係合され、それらを通過して操作ハンドル(17)に接合されているコード(21)により保持されることを特徴とする請求項6記載の用具。
