Abstract

The epiphyseal plate is particularly for osteo-synthesis of bone fractures of the limbs. It is pre-shaped to fit against the bones, its horizontal arm (1a) accommodating screws (4) engaging in the epiphyseal, while its vertical arm (1) has an oblong metaphysical opening (1c) to take an oblique screw (5) for location in the vertical direction. The plate can be designed for use at the level of the upper end of the tibia, its horizontal arm fitting behind the rotulian tendon.

Description

The present invention relates to improvements made to the epiphyseal plates intended for osteosynthesis of fracture of the bones of the limbs. The plates according to the invention are more particularly intended for osteosynthesis or epiphyseal or metaphyseal osteotomies.

There are such plates but their installation is difficult because the surgeon must himself form them so that they follow the shape of the bone. It is easy to understand that this forming is never perfect because first of all of the high resistance of the material from which said plates are made and on the other hand of the considerable efforts that must be developed to carry out these cold deformations.

The present invention aims to allow the production of preformed lateralized epiphyseal plates so that they can cooperate with the epiphyses of the humerus, radius or tibia of the limbs of a human being. The invention also aims to allow the implementation of plates of the kind in question capable of adapting in the vast majority of cases to the shape of the epiphyses considered, their use in certain circumstances allowing the removal of post-operative plaster compression. , so that ultimately it ensures faster rehabilitation of the patient.

The appended drawing, given by way of example, will allow a better understanding of the invention, the characteristics which it presents and the advantages which it is capable of providing.
The single figure of the drawing illustrates the use of two epiphyseal plates according to the invention applied to the upper epiphysis of the tibia. In this figure, some of the fixing screws have not been shown so as not to overload the drawing.

The plates 1, 2 shown in the figure of the drawing each include a horizontal branch la, 2a and a vertical branch lb, 2b. Each of the horizontal branches 1a, 2a cooperates with a part of the epiphysis 3a of the tibia 3, which in fact constitutes the tibial plateau. The horizontal branch of the external plate 1 is placed behind the patellar tendon and its holes allow the placement of screws 4 which are inserted into the epiphysis. Due to the rounded shape of the branch considered, fixing by screws provides excellent horizontal triangulation, that is to say that the bearing forces of this branch on the epiphysis are concurrent. The vertical branches of the plates considered each have an oblong opening Ic, 2c allowing the introduction and the installation of a screw 5, 6 which can be oriented obliquely in order to reinforce the solidity of the assembly the use of the plates considered in the fractures of the extreme tibial plateau provides a console-shaped assembly which increases the solidity of the assembly.

In external compression osteotomies, the displacement can be reduced on the preformed plate.

The internal plates are used for the synthesis of fractures of the internal tibial plate, for the osteotomies of subtraction for genu valgum, of addition for genu varum. In this case the graft is held by the presence of the plate and by the oblique screw 6. The horizontal branch of the plate 2 receives screws 7 similar to those 4.

The upper plates of the tibia also offer the possibility of achieving significant compression during an osteotomy.
Plates similar to those provided for the upper end of the tibia can be produced in order to cooperate with its lower end. The plate then rests by its diaphyseal part (vertical branch) on the anterior-internal side of the tibia and by its horizontal branch on the anterior side of the epiphysis. They are used to treat fractures of the lower quarter of the leg provided that their vertical branches are long enough. The use of such plates at this level is particularly advantageous because the assembly, although solid, leaves the tibio-tarsal joint free. It is also possible to remove the post-operative plaster compression as indicated above.

Of course the screws 5, 6 can be oriented in order to cooperate either with the diaphysis, or with the ltepiphysis of each bone.

It is also possible to produce lower humerus end plates which are used for osteosynthesis of supra, sus or inter-condylar fractures. They are put in place by the transtricipital or transolecranial posterior route.

Dans le cas du radius la plaque ne présente pas la forme d'une équerre mais celle d'un Y. One can also make plates of lower end of radius for osteosyntheses of the anterior marginal fractures, the bones teotomies of vicious cal or resumptions of fractures. The reduction is carried out by the plate. In the case of the radius the plate does not have the shape of a square but that of a Y.

Thus, lateralized preformed epiphyseal plates were produced which are compact, therefore which do not cause any particular discomfort for the patient and which allow the realization of a very solid fixation.

It should moreover be understood that the above description has been given only by way of example and that it in no way limits the field of the invention from which one would not depart by replacing the execution details described by all other equivalents.
