Background & objectives:Galantamine, a centrally-acting cholinesterase inhibitor, has been used i... more Background & objectives:Galantamine, a centrally-acting cholinesterase inhibitor, has been used in the treatment of mild-to-moderate dementia of Alzheimer disease. Increased mortality, mainly due to cardiovascular events, was observed in placebo-controlled trials of galantamine. Several studies have evaluated the efficacy of galantamine in dementia, it is not clear whether it has an effect on platelet function. It is important to clarify this effect, because it may be related to thrombotic tendency or bleeding diathesis. This study was aimed to investigate the effect of galantamine on platelet aggregation in whole blood from healthy, elderly subjects.Methods:Fifteen healthy (mean age 76.8 ± 7.2 yr) volunteers were included in the study. Three concentrations of galantamine solution (20, 40 and 80 ng/μl) were prepared. Each concentration of galantamine solution and control diluent without galantamine were incubated with whole blood. After incubation, aggregation responses were evaluated with ADP (5 μM) and collagen (2 μg/ml) in platelet-rich plasma.Results:Compared to control, pre-incubation with all dilutions of galantamine had no detectable effect on platelet aggregation response induced by ADP and collagen. Galantamine also had no detectable effect on platelet aggregation in a dose-dependent manner.Interpretation & conclusions:This in vitro study suggested that galantamine administration had no effect on platelet aggregation in the clinically relevant doses.

Objective: To examine the association between body mass index (BMI) and cognitive decline (CD) du... more Objective: To examine the association between body mass index (BMI) and cognitive decline (CD) due to Mild Cognitive Impairment (MCI), Alzheimer's Disease (AD), and Vascular Dementia (VaD). Design and setting: The subjects aged ≥ 65 years were recruited prospectively from the Geriatrics Clinic of Gulhane Medical School, between 2004 and 2008 years. Participants: 1302 patients were included in the study. Measurements: Cognitive status, clinical diagnosis of CD (MCI, AD, and VaD) and clinical and environmental risk factors were evaluated by comprehensive geriatric assesment. Finally, the subjects were categorized into two groups according to having CD or not. Results: 905 (69.5%) subjects were not having CD whereas 397 (30.5%) patients with CD. Of the patients with CD, 140 (10.4%) had MCI, 227 (16.9%) AD, and 30 (2.2%) VaD. After adjustment for confounding with a model for multiple regression analysis, age (OR=1.054; CI:1.027-1.083; p<0.001) and family history of dementia (OR=1.662; CI:1.038-2.660; p=0.034) were found to be independent risk factors for CD. Also, overweight (OR=0.594; CI:0.370-0.952; p=0.03) and obese (OR=0.396; CI:0.242-0.649; p<0.001), and high education level (OR=0.640; CI:0.451-0.908; p=0.012) were found to be independent protective factors for CD. Conclusions: We found the risk of CD decreases in overweight and obese elderly. The results indicate that the primary prevention should not only consider risk factors, but must also take anthropometric data into consideration in order to identify persons at high risk for CD.

Bu çalışmalarda eğitim ve iş dünyasının ihtiyaçlarına uygun eleman yetiştirilemediği düşüncesi yaygındır. Göz ardı edilemeyecek olan bu durum, eğitim kurumları dahil, iş dünyası ile eğitim dünyası arasındaki iletişimin yeterince kurulamadığını göstermektedir. O halde bütün meslek sahipleri birden fazla öğretmen tarafından yetiştirildiğine göre yaşadığımız bu yüzyılda öğretmenin yükleneceği misyon ne olmalıdır, bu misyona sahip olması için öğretmenler günümüz eğitim ve öğretim sistemi içinde nasıl yetiştirilmelidir. Bu düşünceden hareketle bu araştırmada; öğretmen yetiştirmede ve öğretmenlik mesleğindeki genel sorunlar literatüre dayalı olarak araştırıldı ve öğretmen yetiştirmede temel noktalar belirtilerek önerilerde bulunuldu.
Background: Disturbances in insulin metabolism, especially insulin resistance, may play a role in... more Background: Disturbances in insulin metabolism, especially insulin resistance, may play a role in most pathogenic processes that promote the development of AD seem conflicting. This study was designed to evaluate if there is a relation between peripheral insulin resistance and cognition in 47 normal subjects and 25 newly diagnosed patients with Alzheimer disease having clinical dementia rating scale (CDR)ϭ 1 at baseline and after receiving galantamine, a central acting acetylcholinesterase Inhibitor, at 6th and 12th months. Methods: Seventy-two consecutive patients age 65 or older without diabetes mellitus were evaluated at a geriatric clinic. Peripheral insulin resistance was calculated with modified-HOMA formula which is suitable for elderly. Serum samples and assessments (MMSE, CDT, CDR, Barthel index, Lawton-Brody IADL, MNA and mobility assessment) were collected at baseline, 6th and 12th months. Results: The mean values of MMSE scores for normal group were 28.00 (SD 1.14), 28.19 (SD 1.38) and 28.11 (SD 1.63), and for Alzheimer group were 21.32 (SD 2.58), 22.36 (SD 2.94) and 22.52 (SD 2.85) at baseline, 6th and 12th months, respectively. The mean values of modified HOMA scores for normal group were 1.611 (SD 0.067), 1.601 (SD 0.048) and 1.596 (SD 0.036), and for Alzheimer group were 1.671 (SD 0.054), 1.601 (SD 0.056) and 1.595 (SD 0.055), at baseline, 6th and 12th months, respectively. Although there is an improvement in MMSE scores of Alzheimer group due to galantamine treatment, there were no statistically significant differences between modified HOMA scores of normal and Alzheimer group and also with in groups. Conclusions: According to the present literature this is the first human study about the possible relation of peripheral insulin resistance and cognition between normal and newly diagnosed patients with Alzheimer disease at baseline and receiving galantamine in prospective manners. To sum up, there may be a difference between peripheral and central nervous system insulin resistances or galantamine, itself, may show its effects apart from action on the insulin-signaling pathway. These results are not enough to make general comments. Further studies are necessary.

Despite the new diagnostic and therapeutic advancements, acute pancreatitis has still high rate o... more Despite the new diagnostic and therapeutic advancements, acute pancreatitis has still high rate of morbidity and mortality. We aimed to evaluate the effects of hyperbaric oxygen (HBO) therapy alone or combined with S-methylisothiourea (SMT), and meropenem (MER) therapy in an experimental rat model of acute necrotizing pancreatitis. Rats were randomly divided into 8 groups, and acute pancreatitis was induced in all groups except group 1. Treatment protocols were saline for group 2, SMT for group 3, SMT + MER for group 4, SMT + HBO for group 5, HBO for group 6, HBO + MER for group 7, and MER for group 8. All surviving animals were killed 48 hours after the induction of pancreatitis, and specimens were collected. Oxidative stress parameters, histopathologic scores and amylase levels were better in treatment groups than in the positive control group (group 2). The most favorable results were obtained in HBO treatment groups, especially in HBO + MER group (group 7). Our results indicate that adding HBO therapy to the antibiotic therapy will decrease oxidative stress parameters, serum amylase levels, and histopathological score. We suggest that adding the HBO therapy as an adjunctive to the treatment protocol of acute necrotizing pancreatitis may yield improvement in the morbidity and mortality of the disease.

In this study, it was aimed to investigate the useful effects of BM-MSCs therapy on the streptozotocin-induced neurodegeneration in rats. Adult female Wistar rats were bilaterally injected intra-cerebroventricularly with streptozotocin (3 mg/kg) for neurodegeneration. Water maze tests were used to monitor spatial learning and memory. One or two intravenous injections of BM-MSCs were administrated to rat via the tail veins. At the end of the study, all rats were sacrificed for histological evaluation and immunohistochemistry. Streptozotocin group demonstrated a significant increase in escape latency in comparison with both control groups (Sham and Saline), whereas rats treated with BM-MSCs exhibited a decrease in escape latency in comparison with streptozotocin group. We aimed to evaluate the effects of hyperbaric oxygen (HBO) therapy alone or combined with S-methylisothiourea (SMT), and meropenem (MER) therapy in an experimental rat model of acute necrotizing pancreatitis. Rats were randomly divided into 8 groups, and acute pancreatitis was induced in all groups except group 1. Treatment protocols were saline for group 2, SMT for group 3, SMT + MER for group 4, SMT + HBO for group 5, HBO for group 6, HBO + MER for group 7, and MER for group 8. All surviving animals were killed 48 hours after the induction of pancreatitis, and specimens were collected. Oxidative stress parameters, histopathologic scores and amylase levels were better in treatment groups than in the positive control group (group 2). The most favorable results were obtained in HBO treatment groups, especially in HBO + MER group (group 7). Our results indicate that adding HBO therapy to the antibiotic therapy will decrease oxidative stress parameters, serum amylase levels, and histopathological score. We suggest that adding the HBO therapy as an adjunctive to the treatment protocol of acute necrotizing pancreatitis may yield improvement in the morbidity and mortality of the disease.

In this study, it was aimed to investigate the useful effects of BM-MSCs therapy on the streptozotocin-induced neurodegeneration in rats. Adult female Wistar rats were bilaterally injected intra-cerebroventricularly with streptozotocin (3 mg/kg) for neurodegeneration. Water maze tests were used to monitor spatial learning and memory. One or two intravenous injections of BM-MSCs were administrated to rat via the tail veins. At the end of the study, all rats were sacrificed for histological evaluation and immunohistochemistry. Streptozotocin group demonstrated a significant increase in escape latency in comparison with both control groups (Sham and Saline), whereas rats treated with BM-MSCs exhibited a decrease in escape latency in comparison with streptozotocin group.
