Ho... more It is widely agreed that obsessive-compulsive disorder (OCD) is less common among the elderly. However, several studies suggest that a third peak of OCD onset may occur after the age of 65. The onset of OCD in the elderly is unusual and mostly related to nonpsychiatric diagnoses. Nonetheless, some reports have documented late-onset OCD in older adults with no detection of cerebral abnormalities. Such differences in age of onset may be associated with phenotypical differences in disease severity, comorbidity, and treatment response across patients. In this report, we describe the case of late-onset OCD in an 80-year old man with no specific focal brain structural abnormality. The report could improve awareness of the disorder in the elderly and contribute to a better identification of clinical characteristics and additional risk factors of OCD.

Introducao: O delirio de infestacao (DI) e caracterizado pela crenca persistente de infestacao po... more Introducao: O delirio de infestacao (DI) e caracterizado pela crenca persistente de infestacao por organismos vivos ou inanimados na ausencia de qualquer evidencia clinica e/ou microbiologica nesse sentido.Objetivos: Descrever quatro casos clinicos de DI e discutir o enquadramento nosologico do DI.Metodos: Relato de quatro casos clinicos de DI observados no Hospital de Braga entre 2010 e 2015 e analise nao-sistematica da literatura baseada na base de dados MEDLINE.Resultados e Conclusoes: A nosologia e a classificacao do DI sao controversas na literatura e para isso contribuiu o facto de nunca se ter estabelecido como entidade clinica independente nos principais manuais de classificacao. A diferenciacao entre um DI primario e independente e um DI secundario, que surge como sintoma de uma outra condicao medica, exige uma avaliacao cuidadosa, existindo multiplas patologias medicas as quais foram associados casos de DI.

Hoarding disorder is characterized by a persistent difficulty discarding items, the desire to sav... more Hoarding disorder is characterized by a persistent difficulty discarding items, the desire to save items to avoid negative feelings associated with discarding them, significant accumulation of possessions that clutter active living areas and significant distress or impairment in areas of functioning. We present a case of a 52-year-old married man who was referred to the psychiatry department for collecting various objects that were deposited unorganized in the patient's house. He reported to get anxious when someone else discarded some of these items. This behavior had started about 20 years earlier and it worsened with time. The garage, attic, and surroundings of his house were cluttered with these objects. On admission, in the mental status examination, it was observed that the patient was vigil, calm, and oriented; his mood was depressed; his speech was organized, logic, and coherent; and there were no psychotic symptoms. Cannabis is the most cultivated and abused illicit drug worldwide. Paradoxically to the antiemeti... more Cannabis is the most cultivated and abused illicit drug worldwide. Paradoxically to the antiemetic properties attributed to cannabis, a relatively new cannabinoid hyperemesis syndrome (CHS) started to be recognized and is characterized by cyclic vomiting that are interspaced by asymptomatic phases. We present a case of a 36‑year‑old woman who repeatedly presented to the emergency room with cyclic vomiting that alleviated with hot showers. She was a long‑term cannabis user and the diagnosis was only established several years later after the onset of symptoms. The diagnostic work up was unremarkable, and the only effective treatment was cannabis cessation. Hot bathing behavior is a key characteristic of this syndrome. CHS is a new clinical condition that should be considered in a setting of recurrent and intractable vomiting in patients with a history of cannabis use.
