Change semantics of attaching promotions to orders

We have two data points that reflect that a promotion has been eligible for an order: The creation of adjustments, and the creation of an entry in order.order_promotions.

I think that entries in that list should exclusively be

• promotions that were attached through a promotion code or
• promotions that were added to the order manually (by an admin, for example).
