To make issues more manageable, I would appreciate it if you fill out the following details as applicable:

General information

  1. Android Version
    8.1
  2. Android Vendor/Custom ROM
    Xiaomi
  3. Device
    MI BOX S
  4. Version of the app (version number/play store version/self-built)
    0.7.8 code 161

Description of the issue

  1. When the first profile is highlighted it takes five or six clicks on the down button of the remote to highlight the second profile. Subsequent up/down clicks move the highlight to the next profile above or below correctly.
  2. The on-screen image is too big and extends out of sight. Yes, the settings of the device can be adjusted to compensate for this but then the other apps don't use the full extent of the screen.

Well the app is not really optimised for Android TV so thing like this are expected. But it also sounds like your TV is cropping the image. My Panasonic/Nvidia Shield combination does not have this clipping problem.

Another UI issue on this same device is the allowed apps page. It takes multiple presses with a usb mouse to check or uncheck the "VPN is used only for selected apps" checkbox. And it's impossible to check or uncheck any apps in the list below

Same issue as the one Wazbat described. Even with a mouse pointer it is impossible to select the checkboxes for the app to include/exclude from the VPN. Philips Tv with Android tv 8.0.0

Latest version made this worse, it was working quite well before the latest updates.

I also have this on my Nvidia Shields - it takes many presses (a large random amount) of the remote to navigate through profiles/connections.

I know of these issues but designing a proper Android TV UI takes a lot of time and effort.
