Allow to add more fields in the petition and to select which to use per-petition #163

For instance have a finite set of available fields like:

first name
last name
phone number
age
nationality
gender
address
Country
zip code / postal code
etc

And allow to select which field you want to appear in your petition.

++1 on that one. This would be a must have for me.
If you implement this, don't forget the zip code (code postal).
