Style guide: branch & PR naming conventions #2107

We should add a few things to the style guide about branch & PR naming:

Branch name should start with the issue #
PR should include (connects #X) where #X = issue number
Add issue # as link (like fix Vagrantfile #1) as the first comment of the PR (do not put "fixes" in this comment because that will automatically close the issue)
