Capture opt-out telemetry

This is a tracking ticket for implementing an anonymous opt-out telemetry tracking system. Medplum is committed to an open and transparent model. "This information is used to shape Medplum's roadmap and prioritize features."

Questions:

• Which apps should send telemetry? app, server, storybook, graphiql?
• Which triggers should send telemetry? only at startup?
• Which modes should send telemetry? dev, production, both?
• Where should telemetry go / how should we collect? Setup https://telementry.medplum.com/ ?
