Document how to set up an app on your custom domain

We give some instructions in the foo medical docs (https://github.com/medplum/medplum/tree/main/examples/foomedical) on how to setup your application on your own domain, but we have nothing in the public docs.

TBD where this doc will go (either self-hosting or tutorials).

Steps to be shown:

• Set up the site settings for a given ClientApplication
• Set up your site settings in Google Console (if using google login) and recaptcha
• Launching your app on Vercel

Doing this as a tutorial follow up to Medplum Hello World makes sesne
