It was observed that for a symmetric 2-layer box model with a 5-spot well development and the multiple perforation option (and a water-oil contact between the 2 layers), the connections are not symmetric. The files to reproduce this behaviour are attached to this issue (note: ocw_depth in yml file is incorrect, should be 2570 instead of 2590).

It is not entirely clear at this moment to what extent this behaviour, as well as other characteristics of Delaunay triangulation (for example, without the central 5th well, there would be only 1 diagonal), will affect the performance of network architectures of real-field FlowNet models.
