add health-related events to adherence plotting

some key events in patient's medical history may be necessary to display in adherence plots, such as diagnosis date, consultations, occurrence/recording of (changes in) health status. to create a separate type of input table for these types of events and include them in the CMA0 plotting (as single-day events, or with specified duration).
