Refresh IG fails when measure or library folder is missing #43

Refresh IG appears to require that the following folders exist, even if no such resources are in the IG.
input/resources/library
input/resources/measure

To repro:

  1. Check out the opioid cds IG repo
  2. Delete everything under input/resources
  3. Run refresh
