  • Six input fields
    • First Name (required)
    • Last Name (required)
    • Email Address (required, email)
    • Phone Number (required, phone)
    • Create Password (required, minimum password requirements)
    • Re-enter Password (required, match)
  • Show Password
  • Submit Button
  • Link to Login Page
  • Validation for all fields
