Better indication of which invocation of a mock had failed expectations #267

If your code under test is calling the same function many times it can be hard to figure out which of the expectations actually failed.

One idea would be to indicate an "index" or "call number" in the error message:

Expected [[name] parameter in [5th invocation] of [normalizeFileName]] to ...
