  1. npi: Unique clinician ID assigned by NPPES
  2. pac_id: Unique individual clinician ID assigned by PECOS
  3. pac_id_org: Unique group ID assigned by PECOS to the group
  4. enroll_id: Unique ID for the clinician enrollment that is the source for the data in the observation
  5. first_name: Individual clinician's first name
  6. last_name: Individual clinician's last name
  7. middle_name: Individual clinician's middle name
  8. org_name: Healthcare organization's name.
  9. gender: Individual clinician's gender
  10. fips: FIPS state and/or county code where the Medicare beneficiary resides. The Bene_Geo_Cd will be blank for data aggregated at the National level or for Puerto Rico and the Virgin Islands.
  11. city: Group or individual's city
  12. county: county
  13. state: Group or individual's state
  14. state: Two-letter state abbreviation of beneficiary residence
  15. zipcode: Group or individual's ZIP code
  16. country: Group or individual's country
  17. med_school: Individual clinician’s medical school
  18. grad_year: Individual clinician’s medical school graduation year
  19. specialty_prim: Primary medical specialty reported by the individual clinician in the selected enrollment
  20. ind_assign: Indicator for whether the clinician accepts Medicare-approved amount as payment in full.
  21. group_assign: Indicator for whether the group accepts Medicare-approved amount as payment in full.
  22. specialty_code: Code that corresponds to the listed Medicare specialty
  23. specialty_desc: Description of the Medicare Provider/Supplier Type
  24. taxonomy_code: The taxonomy codes the providers use
  25. taxonomy_desc: The description of the taxonomy that the providers use
  26. year: YYYY, calendar year of Medicare enrollment. 2007-2018 data is currently available.
  27. level: Geographic level of data; options are National, State, and County
  28. geo_desc: ??Geographic level of data; options are "National", "State", and "County"
  29. age_group: Identifies the age level of the population that the data has been aggregated.
  30. demographic: Identifies the demographic level of the population that the data has been aggregated.
  31. demo_desc: ??Drilldown after demo_level, e.g.
  32. condition: a chronic condition
  33. facility_type: Facilities can fall into the following type categories: Hospital, Long-term Care Hospital, Nursing Home, Inpatient Rehabilitation Facility, Home Health Agency, Hospice, and Dialysis Facility
  34. facility_ccn: Medicare CCN (CMS Certification Number) of facility type or unit within the hospital where an individual clinician provides service. Effective 2007, the CCN replaced the terms Medicare Provider Number, Medicare Identification Number and OSCAR Number. The CCN is used to verify Medicare/Medicaid certification for survey and certification, assessment-related activities, and communications.
  35. parent_ccn: Medicare CCN of the primary hospital where an individual clinician provides service, should the clinician provide services in a unit within the hospital.
