Looking at WP-Polls right now https://github.com/lesterchan/wp-polls as a contender. I'm not confident at this point that I can hook it on to an action that triggers after a form is sent. Will continue to investigate

Could it work/appear as a widget on a forum page (like Workshop Review) set up by Organizers after an event has finished?
