Add "spreadsheet" (matrix) style view of forms and preference option to select between this and the conventional "form" view

Incorporate form representation as a spreadsheet/matrix which dynamically maps fields as column headers and allows multiple data entry as rows. A working example of this was developed by Chris Godwin for the Green Army reporting process.

Allow preference based selection of the required view in survey configuration settings ("Survey form" tab).
